Application form

For a small number of our courses you need to fill in a downloadable application form and send it to us. 

Send your completed application form to new-applications@shu.ac.uk with the full course or module title in the subject line.

Reference form

If you are a referee, you might be asked to fill in a downloadable form.

This could happen if the link on your reference request email expires, or if you are unable to access it for any reason.

Send the completed form to new-applications@shu.ac.uk.
